An elderly patient visited me a few days ago. He had a fistula in his left arm and was on regular dialysis. He was quite fed up getting hooked up to the dialysis machine 3 times a week! His mind was eager to skip the dialysis. A thought came to his mind! "Maybe the kidneys aren't dead completely, and he can live without dialysis". He decided to check his kidney status one last time.
I tried to convince him not to do the tests but he insisted. So, I suggested some investigations. He was still passing urine somewhat. I advised serum creatinine, urea, urine microscopy, and eGFR. The results were indeed bad. The eGFR was only 6 (normally is it above 90). When the eGFR value is less than 15, we consider it as ESRD (end-stage renal disease). That means his kidney was indeed at its end stage and he was completely a dialysis-dependent patient.
I informed him of the results over WhatsApp. He received, read, and replied to the message. He didn't visit me in my chamber afterward!
