It’s a norm in Bangladesh to hide a serious disease such as cancer from the patient. Whenever one or more of the family members bring a patient to a doctor, directly or indirectly they inform the doctor not to divulge the disease to the patient. Rather, they ask to tell them first. There is some debate about this issue. One of my teachers was strongly against it. According to that teacher, it’s unethical to divulge a patient’s information to anyone other than the patient without the patient’s permission, even to the spouse! Others think that patients may not adequately handle bad news. So, involving family members is justified. What’s your opinion on this? Now let's go to today’s chamber talk.

An above-60 patient was brought by his son. Quite a caring son! Before talking with the patient about his illness, the son talked with me privately outside of my chamber. It was a cancer patient with metastasis to multiple body parts! He couldn’t talk about primary cancer. The family members haven’t told the patient that he has cancer, which already has spread to other body parts! So, the son requested me not to talk about cancer with the patient!

Besides cancer, he had a chest infection and left inguinal area abscess-like wounds. He was a typical picture of cancer Cachexia (lean, thin, emaciated!). There was nothing much to do for him. I just gave supportive and symptomatic management! He was sick but well-oriented!

The word cancer brings extreme fear to the patients and their family members. So, I think involving others when breaking the bad news is justified. But, hiding such a disease from the patient is also not the correct method!

I believe letting the patient know his or her diagnosis is good.
So, whenever the treatment involves you will be able to show concern to respond to it.
Just like when you are advised to abstain from eating this or that.
You should know and take the consequences serious.

I also agree with you in this regard. I prefer informing patients, with or without the presence of relatives. They should learn to handle the issue by themselves.

I think first and foremost the person with cancer should be the first to know, what if they need to get affairs in order but did not know they had anything bad wrong, secondly, if they want you to tell others, it should be up to them to do so or ask you to inform them on a visit to you.

I would be very upset if everyone knew but me.
